Adding the Acknowledgement field to your form is useful when you need to ensure that the respondent has read and understood a particular statement or agreement. This field requires the user to actively acknowledge the statement or agreement on the form before they can submit it.
The Acknowledgement field is a great way to make sure your workers have read and agree to certain terms outlined in any important document, such as:
- Safety procedures
- Policies and guidelines
How to Add an Acknowledgement Field
- In the Form Builder, select the Acknowledgement field from the Add Fields menu on the left.
- In the Acknowledgement Settings menu on the right, enter the information that the user is acknowledging in the Value box.
- Select Configure Button Text to change the text of the acknowledgment button that the user will click on (the default text is I Agree and Thank You.)
Once your form is published, your users will be required to acknowledge the information that you typed before they can submit the form.
